    Department of Family Medicine faculty provides comprehensive family-centered care in community-based Family Medicine Practices, residency program health centers, and UPMC hospitals.

    The Family Medicine Practices follow a community health center, private practice or federally-qualified health center (FQHC) model. The FQHC within the Department is the only such access point within UPMC and provides comprehensive, family centered care (both ambulatory and inpatient) to enrolled patients across the lifespan, to include the care of children, pregnant women and the elderly. The majority of these clinical settings are located in traditionally underserved communities. Department clinical faculty and residents devote tremendous time and effort to providing the highest quality, affordable primary care, and preventive health services to those previously unable to afford or access care.

    The Bloomfield–Garfield Health Center is a comprehensive primary care health center that meets the needs of the community while serving as a clinical teaching site for the UPMC St. Margaret Family Medicine Residency. Dr. Ann McGaffey serves on the neighborhood board and is integrally involved with community outreach.

    The Family Medicine residents and attendings at the Latterman Family Health Center have been serving the medical needs of families in the Mon Valley area since 1974. Multiple board certified specialists including an allergist, geriatrician, ob/gynecologist, pediatrician, psychiatrist, and surgeon work together with the family physicians to provide comprehensive medical care to patients of all ages. A behavioral scientist is available daily to assist the physicians with counseling for emotional, personal, marital, and family issues. There are also various transportation programs for children, pregnant women, adults, and the elderly.

    The center is a CMI practice and multiple process improvement projects help to improve the quality of care. The center participated in various programs and studies such as the Primary Prevention of Diabetes Program, Reach Out and Read, Medical Home Project and a special grant funded program with local boys and girls clubs on lifestyle choices and prevention of diabetes. Research studies included a multi-center study on causes of low birth weight and a study on cognitive impairment. There were numerous community activities under the leadership of Ms. Nina Tomaino.

    The Lawrenceville Family Health Center provides comprehensive family care through community collaborations, school partnerships and a variety of social services supported by the St. Margaret Foundation. The School Asthma Initiative carris out a school-wide “Asthma Awareness Day” at McCleary Elementary school and continues to educate school students with asthma as well as teachers and administrators in 5 school partners. In addition, we provide health education services and physical exams at the Neighborhood Academy and Career Connections Charter High School. We are also able to provide; free adult immunizations to the uninsured/underinsured, bike helmets, emergency medicines, transportation, and translation services for our refugee populations, and other vital social services thanks to the generous support from the St. Margaret Foundation. Lastly, the family health centers affiliated with St. Margaret, Shadyside and McKeesport worked together to secure funding for Reach Out and Read-a national literacy advocacy campaign for children. All of our providers and staff were trained and work to improve literacy and foster a love of reading in our children and families. These activities and numerous others support our mission to provide excellent, comprehensive, family-centered care and to improve the lives of our patients and the communities in which they live.

    The Matilda Theiss Health Center is a UPMC hospital-based clinic, owned and operated by the University of Pittsburgh Medical Center. This Center serves a primarily poor minority population. It is located in a neighborhood with a large proportion of public housing units. The Theiss Center is the only federally qualified health center within UPMC. Primary health care across the life span is provided and overseen by the faculty of the Department of Family Medicine. Full primary care services are offered to include periodic child and adult physical examinations, chronic disease screening and management, immunizations, health education, minor surgery, and obstetrical and gynecological care. The Center also provides dental care, Alcoholics Anonymous meetings, and a medication dispensing service. A variety of social, counseling, and health promotion services, which include a seniors’ health group, are provided by a licensed social worker. The Health Center is a teaching site for first- and third-year medical students of the University of Pittsburgh, as well as St. Margaret’s Family Medicine residents fulfilling their community health rotations.

    The UPMC New Kensington Family Health Center was founded in 2000 to serve the New Kensington community and as a clinical teaching site for Family Medicine residents and fellows from UPMC St. Margaret. Since its inception, the New Kensington Family Health Center has more than tripled its patient volume. The staff provides comprehensive ambulatory care across the life span. We work with many community agencies to provide full spectrum primary care, including maternity care, to our patients; we provide on-site mental health and substance abuse counseling through a collaboration with Family Health Services of Pennsylvania, and we also provide free women's health care, including family planning and colposcopy, to underserved patients through a collaboration with Family Health Council of Western Pennsylvania. The staff was instrumental in identifying and helping contain a major syphilis outbreak that occurred in our community four years ago and continues to collaborate with the Pennsylvania Department of Health to provide free STD screening and treatment. In an effort to promote early childhood literacy, we partnered with Reach Out and Read in 2001, to provide free age-appropriate books to preschool children during well child visits. We extended this collaboration with Reading is Fundamental in 2005, in order to provide books and promote literacy to children in grades one through six. We are working to extend this collaboration with both Reach Out and Read, and also Reading Is Fundamental, through the Family Health Centers at Lawrenceville, Bloomfield–Garfield, Shadyside, and Latterman. Through this spirit of collaboration and community-oriented primary care, we strive to meet the needs of the community in the New Kensington area.

    The Family Health Center at UPMC Shadyside provides a well-organized and comfortable setting for the practice experience. The center serves many diverse sectors of the Pittsburgh community. Families of the poor and disenfranchised receive personalized, comprehensive care; families of graduate students from nearby universities provide an international perspective; and neighbors from the surrounding communities prefer the concern, availability, and continuity of care that are standard practice at the Family Health Center. The center maintains special relationships with many nearby service organizations and facilities, including two different school systems, Healthy Beginnings Plus, the Pittsburgh Women’s Shelter, organizations serving the gay community, Alpha House Drug and Alcohol Rehabilitation programs, and Chatham College. These well-established relationships assure residents of the diversity of experience essential to becoming well-rounded physicians, without the investment of time and effort involved in going outside the residency program.

    This office is one of three in the Department of Family Medicine faculty practice and serves patients with a broad payor mix, from self-pay and medical assistance to HMO and traditional indemnity. We offer care across the spectrum of ages from birth to elderly patients and non-English speakers; we admit to a local nursing home and offer home visits to the homebound as needed. The office is a teaching site for medical students of the University of Pittsburgh, there is at least one third-year medical student in the office monthly, and, often, first-year and second-year students as well.
